At the time of this writing, over 11 million individuals have been affected worldwide (India is fourth among all countries, 674,515) and over half a million people have died. The asthma treatment ventolin has been an unprecedented global stressor, not only because of the disease burden and mortality but also because of economic upheaval.
The very fabric of the society is disrupted, affecting housing, personal relationships, travel, and all aspects of lifestyle. The overwhelmed health-care system is among the most major stressors, leading to a heightened sense of vulnerability. No definitive treatments or treatment is on the horizon yet. Psychiatry has to brace up to an expected mental health crisis resulting from this global stressor, not only with regard to treating neuropsychiatric consequences but also with regard to developing preventive approaches and building resilience.Thankfully, there is a wealth of wisdom to help us in our ancient scriptures such as the Bhagavad Gita[1] for building psychological resilience. The Bhagavad Gita is a dialog between the Pandava prince Arjuna and his charioteer Krishna in the epic Mahabharata, the great tale of the Bharata Dynasty, authored by Sage Vyasa (c.
4â5 B.C.E.). The dialog occurs in the 6th chapter of the epic and has over 700 verses. In this epic story, Arjuna, the righteous Pandava hero was faced with the dilemma of waging a war against his cousins, the Kauravas, for territory. Arjuna is confused and has no will to initiate the war. In this context, Krishna, his charioteer and spiritual mentor, counsels him.
The key principles of this spiritual discourse in the Gita are embodied in the broad concept of yoga, which literally means âYogâ or âto unite.â Applying three tenets of yoga can greatly help developing resilience at individual, group, and societal levels. A fourth path, Bhakti yoga, is a spiritual approach in the Gita which emphasizes loving devotion toward a higher power or principle, which may or may not involve a personal god. In this editorial, I focus on three paths that have considerable relevance to modern approaches to reliance-focused psychotherapy that may be especially relevant in the asthma treatment era. Path of Knowledge The first concept in the Gita is the path of knowledge (Jnana Yoga, chapter 2). The fundamental goal of Jnana yoga is to liberate oneself from the limited view of the individual ego, and to develop the awareness of one's self as part of a larger, universal self.
Hindu philosophers were among the earliest to ask the question of âwho am Iâ and concluded that the self is not what it seems. The self as we all know is a collection of our physical, mental, and social attributes that we create for ourselves with input from our perceptions, and input by our families and society. Such a world view leads to a tendency to crave for the âIâ and for what is mine, and not consider the âWe.â As Krishna in the Bhagavad Gita points out, the person who sees oneself in others, and others in oneself, really âsees.â Such awareness, which guides action in service of self as well as others, is critically important in our goals of collectively preventing the spread of the asthma. A glaring example is the use of face masks, known to effectively slow the viral . Using the mask is as important to protecting oneself from the ventolin as well as protecting others from oneself.
Nations such as the USA (and their leaders), who have given mixed messages to the public about the need to wear masks, have been showing a strikingly high number of cases as well as mortality. Krishna's counsel would be that the doctor should continue to perform his/her own dharma, but do so without desire or attachment, thereby performing action in the spirit of Karma yoga. Such action would be with detachment, without a desire for personal gain and being unperturbed by success or failure. Such âNishkaama Karmaâ (or selfless action) may help doctors working today in the asthma treatment outbreak to carry forward their work with compassion, and accept the results of their actions with equanimity and without guilt.
Krishna points out that training one's mind to engage in selfless action is not easy but requires practice (Abhyasa). Krishna is also emphatic about the need to protect oneself, in order to be able to effectively carry out one's duties. Path of Meditation The third core concept in the Gita is the path of meditation and self-reflection (Raja yoga, or Dhyana yoga, chapter 6). It is considered the royal path (Raja means royal) for attaining self-realization, and often considered the 8-fold path of yoga (Ashtanga yoga) designed to discipline lifestyle, the body and mind toward realizing mindfulness and self-reflection. These techniques, which originated in India over two millennia ago, have evolved over recent decades and anticipate several approaches to contemplative psychotherapy, including dialectical behavior therapy, acceptance and commitment therapy, and mindfulness-based stress reduction.[3] These approaches are of particular relevance for stress reduction and resilience building in individuals faced by asthma treatment-related emotional difficulties as well as health-care providers.[4]The majority of people affected by the asthma treatment ventolin recover, but about 20% have severe disease, and the mortality is around 5%.
Older individuals, those with obesity and comorbid medical illnesses such as diabetes and lung disease, are particularly prone to developing severe disease. "People who have received a cancer diagnosis are faced with a number of challenges, such as accessing reliable and affordable care, that can add to existing anxiety or depression associated with their illness," said lead author Ryan Suk.
"But those who live in economically or medically underserved areas can be especially affected." Suk is an assistant professor of management, policy and community health at the University of Texas Health Science Center at Houston. Her team's analysis of U.S. National Cancer Institute data found that of nearly 5.4 million people diagnosed with cancer between 2000 and 2016, about 6,300 died of suicide. Most patients in the study were men (51%), white (72%), and half were over 65, according to the report published online Oct. 19 in JAMA Network Open.
Compared to the general population, all cancer patients had a higher risk of suicide, the study authors noted. But the risk was greatest for patients in counties with the lowest incomes (1.94 times higher) and in rural counties (1.81 times higher), compared to those in the highest income (1.3 times higher) and urban counties (1.35 times higher). Overall, suicide risk was highest in the first year after a cancer diagnosis and it decreased over time. But the risk of suicide among cancer patients in the lowest-income counties remained 1.83 times higher than that of the general population more than a decade after diagnosis, the findings showed. The researchers said that suggests that financial stress may progressively worsen among cancer patients in the poorest counties.
One finding that stood out to the investigators. The suicide risk among Asian and Pacific Islanders with cancer was higher across all income levels, and even those in the highest-income counties had a nearly doubled risk of suicide compared to the general population. That suggests the need to provide more information about mental health services to Asian and Pacific Islanders with cancer, according to Suk. "Patients living with cancer and cancer survivors face many complex challenges, particularly those living in rural or low-income areas," she said. 21, 2021 (HealthDay News) Nine of 10 patients with so-called "bubble boy" immune disease who received gene therapy about a decade ago are still disease-free, researchers report. The gene therapy was developed at the University of California, Los Angeles (UCLA), to treat the rare and deadly immune system disorder formally known as adenosine deaminaseâdeficient severe combined immunodeficiency (ADA-SCID). It's caused by mutations in the gene that creates the ADA enzyme, which is crucial for a normally functioning immune system. Exposure to common germs can be fatal for babies with the disease. If untreated, most die within the first two years of life.
With the UCLA gene therapy, blood-forming stem cells are removed from a child's bone marrow and a specially modified ventolin is used to place healthy copies of the ADA gene into the stem cells' DNA. The cells are then transplanted back into the child's bone marrow. The therapy is meant to prompt the body to produce a continuous supply of healthy immune cells that can fight s. The transplanted stem cells are the child's own, so there is no risk of rejection. Now, in the study published Oct.
14 in the journal Blood, the UCLA researchers reported that nine of the 10 children who received the one-time treatment between 2009 and 2012 as part of a phase 2 clinical trial remain stable. Most of the children were babies at the time they received the gene therapy. The only one whose immune function wasn't restored by the gene therapy was 15 at the time of treatment. "What we saw in the first few years was that this therapy worked, and now we're able to say that it not only works, but it works for more than 10 years," senior study author Dr. Donald Kohn, with the Eli and Edythe Broad Center of Regenerative Medicine and Stem Cell Research, said in a UCLA news release.
"We hope someday we'll be able to say that these results last for 80 years." Gene therapy for ADA-SCID is not yet approved by the U.S. Food and Drug Administration. Without it, children with ADA-SCID have to receive twice-weekly expensive and time-consuming injections of the ADA enzyme, or receive a transplant of healthy stem cells from a matched bone marrow donor. In this study, patients with more copies of the ADA gene in more cells had the best immune function. More research is needed to determine the best way to achieve high levels of the gene in all patients, according to Kohn, a distinguished professor of microbiology, immunology and molecular genetics.
"What these results tell us is that there's a formula for optimal success for ADA-SCID, and it involves correcting more than 5% to 10% of each patient's blood-forming stem cells," he said. "Knowing that a gene therapy can have this lasting effect in ADA-SCID for more than a decade is important for our path forward as we develop new gene therapies for this and other diseases," Kohn added. In all, 84% of the 269 respondents said NSAID-acetaminophen combos are as effective as opioids or even more so, but 43% also said they regularly prescribe opioid medications. The findings were published Oct. 21 in the Journal of the American Dental Association. "These results suggest that dentists are familiar with the evidence about the effectiveness of NSAID-acetaminophen medications, but their self-reported prescribing patterns demonstrate a disconnect," said first author Matthew Heron, who conducted the study as an undergraduate at Georgetown University's School of Nursing and Health Studies in Washington, D.C. He spoke in a Georgetown news release.
Previous studies have found that dentists represent 8.6% of opioid prescribers in the United States, and are the biggest prescribers of opioids to patients 18 and younger. "We know that the first exposure to opioids for many people occurs in their teens and early 20s following common dental procedures like third molar extractions," said study co-author Nkechi Nwokorie, who also conducted the work as a Georgetown undergrad. "This is a particularly vulnerable population for misuse." Dr. Adriane Fugh-Berman is director of PharmedOut, a Georgetown project that was involved in the study. "This underscores the need for more education about the harms of opioids and the need for national guidelines to align clinical practice with current evidence," she said.

In all, 84% of the 269 respondents said NSAID-acetaminophen combos are as effective as opioids or even more so, but 43% also said they regularly prescribe opioid medications.
The findings were published Oct. 21 in the Journal of the American Dental Association. "These results suggest that dentists are familiar with the evidence about the effectiveness of NSAID-acetaminophen medications, but their self-reported prescribing patterns demonstrate a disconnect," said first author Matthew Heron, who conducted the study as an undergraduate at Georgetown University's School of Nursing and Health Studies in Washington, D.C. He spoke in a Georgetown news release.
Previous studies have found that dentists represent 8.6% of opioid prescribers in the United States, and are the biggest prescribers of opioids to patients 18 and younger. "We know that the first exposure to opioids for many people occurs in their teens and early 20s following common dental procedures like third molar extractions," said study co-author Nkechi Nwokorie, who also conducted the work as a Georgetown undergrad. "This is a particularly vulnerable population for misuse." Dr. Adriane Fugh-Berman is director of PharmedOut, a Georgetown project that was involved in the study.
"This underscores the need for more education about the harms of opioids and the need for national guidelines to align clinical practice with current evidence," she said.
